Please complete the following form if you want your child to participate in distance learning.
Please note that once you have completed this agreement, returning to the school building for instruction will not be allowed until the next 9 weeks grading period.  Students will not be allowed to go between distance learning and face-to-face instruction within a 9 week grading period.  After school extracurricular activities will be allowed with transportation to and from campus being the responsibility of the parent/guardian.

Please complete a separate form for each child you plan to enroll in distant learning.
